Hypotheses

The project primarily addresses the question "Which interventions through General Practice facilitate uptake of chronic disease self-management strategies?"

The main objectives are:

1. To evaluate the impact of the intervention on health outcome measures in patients with poorly controlled T2D (HbA1c>7.5%), compared with usual care only.

2. To estimate the cost of the intervention and cost-effectiveness (relative to usual care only), when implemented in the General Practice setting in patients with T2D.

3. To determine the barriers and facilitators to the implementation of the intervention in General Practice.
